Well I had to look this up for peanut head, and I found that it refers to people with a peanut-shaped head, that is to say people whose head is characterized by a domed shape, and the expression comes from Mexico.
In my family there is no person with this type of head formation, but I remember once a friend called one of my cousins peanut head in a derogatory way.
The expressive and gestural form had a reading for me, and it was that this person was telling my cousin that he was unintelligent.
