When I started the latest psychiatric medication I have been using to treat anxiety and panic, I reported to my doctor that strangely I already had an absurd improvement, almost similar to amphetamine on the first day of use, with 25mg of Clomipramine (one dose media for treatment starts to show results at 75mg). I remember waking up with a smile on my face, energetic, attentive, lucid and motivated, in a way I hadn't seen for months. And that gave me a very strong impression of the medicine's potency. But later, talking to the psychiatrist he said:
-Maybe you won't believe what I'm going to say, but even so, I think it's interesting for you to know. This effect that you have had from day one is a Placebo effect. The medicine will only act on your body clearly after at least 4 weeks.

To me, the Placebo effect is nothing new, but I have never experienced a placebo experience personally and on such an intense level. And this made me understand the potential of this effect not only as a fascinating "anomaly" within the expected accuracy of average science, but also as a possible method of healing that unfortunately we still don't have access to with efficient understanding. So I decided to write a little about it here making a brief analysis of the topic.

In science, placebo is generally not considered a validated method of treatment. Not because it doesn't work or doesn't generate expected effects, but precisely because it is uncertain and fluctuating in terms of results. There is no real individual control over the experience of "unconscious belief" generated by an inert substance. And by the way, it's good to start here, what is Placebo anyway? Placebo is a term used for the effect obtained through effects generated in a patient who consumed a neutral substance, believing it to be a drug. Like the classic "flour capsule" example. But the term extends to every phenomenon that involves the same mechanics. As in the case I reported, where I felt intense stimulating and euphoric effects of the medicine even before it had reached the minimum time to act in my body. Despite not having an accepted therapeutic value as a method of treatment, Placebo is so respected in science that it has become completely tied to the most reliable studies, where 50% of patients in any new scientific study of a drug or treatment are using placebo instead of rather than the substance in question. In addition to this "belief" effect, the inert substance serves as a control equivalent to "the patient's natural cure", a way of watching the body react to the problem on its own and its benefits, when compared to the actual drug administered to the other half of the patients. tested. And as the tests themselves demonstrate, often, almost always, those patients who unknowingly ingested a placebo report specific symptoms and sensations that are abnormal to their ordinary state, thanks to the fact that they think they received the real drug and not the placebo! Soon, Placebo became the control system of science.

The best results ever observed in terms of Placebo are mainly linked to the reduction of pain, insomnia and anxiety and depression. Note that these are always situations well perceived by patients, these are not interventions that occur without the patient's understanding, there are few cases where something extraneous to their participation and cognition can have good results through Placebo. Given that both pain, insomnia, and depression are linked to neurotransmitter mechanisms being more or less used and received by the synaptic cleft, we can conclude, although without much guarantee in this statement, that in an uncontrollable way, we have the potential to change our neurochemistry efficiently to obtain the expected results only with the belief itself, however, unfortunately the process is not simple when it comes to trying the same result with the "intention". It is as if, when we use reason to say: "-I want to produce more serotonin!" we would try to give an order by shouting at a computer instead of just hitting "enter" in your app that would make the solution happen. We do not understand the method by which to make these "natural cures" accessible, due to the lack of control of our neurological system.

I believe neurological science will evolve into many other areas before we can have any more advanced understanding of how to emulate the placebo benefits of purposeful and indifferent "belief". Until then, this curious and important anomalous effect in investigative science will continue its primary role of serving as a control protocol in research. But even though this is its most useful and proven function, there is no lack of examples from everyday life and from our culture that prove that Placebo has always been present in everyday life, such as in Homeopathy, in Florals, in cures pseudo-spirituals, in religious belief systems, and so on!

Para mim, o efeito Placebo não é novidade, mas eu nunca havia vivenciado uma experiência de placebo pessoalmente e num nível tão intenso. E isso me fez compreender o potencial desse efeito não só como uma fascinante "anomalia" dentro da exatidão esperada na ciência média, mas também como um possível método de cura que infelizmente ainda não temos acesso com eficiente compreensão. Decidi então escrever um pouco à respeito disso por aqui fazendo uma breve análise do tema.

Na ciência, de forma geral o placebo não é considerado como método de tratamento validado. Não por que não funcione ou não gere efeitos esperados, mas exatamente por ser incerto e flutuante em termos de resultados. Não existe um controle real individual sobre a experiência de "crença inconsciente" gerada por uma substância inerte. E aliás, é bom começar por aqui, o que é o Placebo afinal? O Placebo é um termo utilizado para o efeito obtido através de efeitos gerados em um paciente que consumiu uma substância neutra, acreditando se tratar de um medicamento. Como o clássico exemplo da "cápsula de farinha". Mas o termo se estende para todo fenômeno que envolve a mesma mecânica. Como no caso que relatei, onde senti intensos efeitos estimulantes e euforizantes do medicamento antes mesmo de ele ter alcançado o tempo mínimo para agir em meu corpo. Apesar de não ter um valor terapeutico aceito como método de tratamento, o Placebo é tão respeitado na ciência que tornou-se atrelado completamente aos estudos mais confiáveis, onde 50% dos pacientes de qualquer novo estudo científico de medicamento ou tratamento, está utilizando placebo ao invés da substância em questão. Além desse efeito de "crença", a substância inerte serve como um controle equivalente a "cura natural do paciente", uma forma de observar o corpo reagir ao problema por si mesmo e seus benefícios, quando comparado ao real medicamento administrado à outra metade dos testados. E como os próprios testes demonstram, muitas vezes, quase sempre, esses pacientes que ingeriram placebo sem saber, relatam sintomas e sensações específicas anormais ao seu estado comum, graças ao fato de acharem que receberam o medicamento verdadeiro e não o placebo! Logo, o Placebo se tornou o sistema de controle da ciência.

Os melhores resultados já observados em termo de Placebo estão ligados principalmente à diminuição de dor, insônia e ansiedade e depressão. Note que são sempre situações bastante percebidas pelos pacientes, não se trata de intervenções que ocorrem sem a compreensão do paciente, são poucos os casos onde algo alheio a sua participação e cognição pode ter bons resultados pelo Placebo. Dado que tanto dor, quanto insônia, quanto depressão estão atrelados a mecanismos de neurotransmissores sendo mais ou menos utilizados e receptados pela fenda sináptica, podemos concluir ainda que sem muita garantia nessa afirmação, que de uma forma não controlável, temos o potencial de alterar nossa neuroquímica eficientemente para obter os resultados esperados apenas com a própria crença, porém, infelizmente o processo não é simples quando se trata de tentar o mesmo resultado com a "intenção". É como se, quando utilizamos a razão para dizer: "-Quero produzir mais serotonina!" nós tentássemos dar uma ordem gritando para um computador ao invés de apenas apertar "enter" em seu aplicativo que faria a solução acontecer. Não compreendemos o método pelo qual tornar essas "curas naturais" acessíveis, pela falta de controle do nosso sistema neurológico.

Acredito que a ciência neurológica irá evoluir em muitas outras áreas antes que possamos ter alguma compreensão mais avançada de como emular os benefícios do placebo propositalmente e indiferentes da "crença". Até lá, esse curioso e importante efeito anômalo na ciência investigativa continuará seu papel principal de servir como protocolo de controle nas pesquisas. Mas ainda que este seja sua função mais útil e comprovada, não nos falta exemplos do dia a dia e da nossa cultura que comprovem que o Placebo sempre esteve presente no dia-a-dia, como por exemplo na Homeopatia, nos Florais, nas curas pseudo-espirituais, nos sistemas de crença religiosa, e assim por diante!

I really enjoyed your post. The placebo effect is real! I've seen it happen with my parents where they take solutions that are of zero value but they think it makes them feel better and they actually become better.
